*NEW STUDENT* Enrollment & Registration Process

OPEN ENROLLMENT BEGINS January 15, 2024.
 
 
Dear Parent/Guardian, 
 
Thank you for showing interest in our school! We strive to offer the best education for children in a safe and secure environment. 
We encourage potential families to visit our school to see what our campus looks like and to ask any questions.  This is not a requirement or a condition to enroll your student at our school.  
To become familiar with our Enrollment & Registration policies, please click here to view our Admission Policy.  If you would like to begin the process of enrolling your child in our school please follow these steps:
      
     1.  Enrollment:
 
To complete this process, you must complete the Enrollment form and the Arizona Residency Form including one form of the documentation which bears the parent or legal guardian's full name and residential address.
 
 
 
Once an enrollment form has been completed and a seat has been confirmed and available, the registration forms can be filled out by the Parent/Guardian.
 
     2.  Registration:
 
Parents/Guardians will be given the Registration forms to complete prior to the student's first start of school.  Please make sure all areas are completed, including any signatures required.
Before a student's first day of attendance, the following documentation must be provided:
  •    Official Notice of Pupil Withdrawal - A student who has previously attended a school in Arizona is required to submit a withdrawal form from their previous school.
  •    Immunization Records - Most types of records supplied by the health care provider are acceptable, as are records supplied by a previous school or childcare center.  Pursuant to Arizona law, "[a] pupil shall not be allowed to attend school without submitting documentary proof to the school administrator unless the pupil is exempted from immunization pursuant to  A.R.S. ꔧ 15-872. Please contact the registrar if you have any questions about exemption forms or other documentation that can be submitted.
 
Other documentation needed to complete the registration process are as follows:
 
Proof of Age/Identity - (Within the first 30 days of enrollment) All Arizona schools must obtain age and identity documentation for students from the Parent/Guardian.  Acceptable proof of age/identity documentation are as follows:   
  1. A certified copy of the student's birth certificate                                                                                               
  2. Other reliable proof of the student's identity, including a baptismal certificate, an application for a social security number, or original school registration records.  This documentation must be accompanied by a notarized affidavit explaining the inability to provide a copy of the birth certificate.
  3. If a student is in the custody of the Department of Child Safety (DCS), a letter from an authorized representative of the agency certifying that the student has been legally placed in custody of the agency as prescribed by law.
Free and Reduced Lunch Application - Families may be eligible to receive benefits under the Federal Child Nutrition Program.  To determine eligibility, Parents/Guardians must complete the Application for Free and Reduced Price Meals which will be available after July 1 of the school year for which it is intended.
 
Home Language Survey Form - (Included in registration forms)  The responses to this form are used by the school to provide the most appropriate instructional programs and services for the student.

The records or documents mentioned above will not be used in making admission or enrollment decisions other than in situations where a student has been expelled or is in the expulsion process.
